USING GRACE SPACES
Use this form to apply for use of any space operated by Grace Church (i.e. Parish Hall, sanctuary, All Saints Chapel, playground, etc.)
Please describe what type of event is planned (wedding reception, community education, staff meeting, etc.) and any details you may feel are helpful in evaluating this application.
For food, if you are using a caterer, specify name, address and phone number.
For alcohol, specify type and circumstances.
If you will be using music, will it be live or recorded? List name, address and phone number of music provider. Include any other details you feel may be useful in the review and evaluation of this application.
I understand that either a copy of the organization's or individual's insurance liability policy with Grace Church named as an additional insured entity OR a Hold Harmless Agreement must be on file with this application if the planned event is approved.*
I have read the Regulations governing the use of the requested space(s) offered by Grace Episcopal Church. This application is in accordance with those regulations. All persons assisting me in this event have been made aware of the regulations.*
If a security deposit or any fee is required in the Regulations, they will be paid to the Parish Administrator under the terms of the regulations.*
I agree to provide the required insurance documentation under the terms of the Regulations.*
